Our Faculty

Cinthia Elkins, MD, PhD is a graduate of the Medical Scholars Program at the University of Illinois in Champaign-Urbana where she completed an MD as well as a PhD in Community Health with a focus in health policy. She completed her training at St. John Providence Family Medicine Residency with low-risk OB/GYN. She is a member of the American Academy of Family Physicians, the Society for Teachers of Family Medicine, and the North American Primary Care Research Group. She also holds a Bachelors degree in Cultural Anthropology. Dr. Elkins sees men, women and children of all ages. She has a special interest in women's health, prenatal and obstetrical care, preventative medicine and health maintenance. She believes in providing a patient-centered medical home where a long-term, trusting partnership enables maintaining optimal health and healing from the occasional hard knocks life may throw at us.
Gary Falkenberg, DO is a graduate of Des Moines University College of Osteopathic Medicine and has practiced in the field of family medicine for over 36 years. Dr. Falkenberg is the Program Director for the Osteopathic Family Medicine Residency at St. Joseph Mercy Hospital Livingston. He is a lifetime member of The American Osteopathic Association, The Michigan Osteopathic Association, and the Livingston County Osteopathic Association. Dr. Falkenberg is a patient advocate providing necessary care and establishing relationships with entire families in his local area.
Joseph Giannola, MD is a graduate of the American University of the Caribbean School of Medicine. He completed his residency in family medicine at Providence Hospital in Southfield. Dr. Giannola is the Associate Program Director for the St. Joseph Mercy Hospital Livingston Family Medicine Residency Program. He is board certified in family medicine and is a member of the American Academy of Family Physicians. He has a special interest in electronic health information technology. Joseph Giannola, MD
Susan Graham, MSW graduated from the University of Michigan School of Social Work. She has a variety of experience both in clinical and macro practice. She has a special interest in resident wellness and global health. Ms. Graham is a member of the National Association of Social Workers and the Society for Teachers of Family Medicine.
Jennifer Larson, MD is a board certified family physician with over ten years of experience. She sees patients of all ages and has a special interest in women's health, geriatric medicine, and the care of adolescents. Dr. Larson earned her medical degree at the University of Maryland School of Medicine and completed her residency training at York Hospital in Pennsylvania. She is a member of the American Academy of Family Physicians, Michigan State Medical Society and the Christian Medical and Dental Associations.
Sanford Lax, MD is a native suburban Detroit Family Physician. He holds a Bachelors of Science in Cellular, Micro, and Molecular Biology from the University of Michigan and completed his medical education at Wayne State University. He completed his residency training at Providence Hospital. His current areas of interest are preventative medicine, athletic training with aging, and teaching. Dr. Lax is board certified and a member of the American Academy of Family Practice.
Nicole Rothenberg, MD is a graduate of Wayne State University School of Medicine. She completed her residency training in family medicine at Providence Hospital. As Chief Resident she was presented the Teaching Excellence Award and the Michigan Academy of Family Physicians Research Award. Dr. Rothenberg provides primary care for family members of any age, with an emphasis on health maintenance and preventative care. Her special interests include obstetrics, women's health care, and food allergies. Her professional affiliations include the American Academy of Family Physicians and the Michigan Academy of Family Physicians.
Joshua Smith, MD is a graduate of the University of Michigan School of Medicine. He also completed his residency training in family medicine at the University of Michigan. Dr. Smith is the Director of Inpatient Medicine for the Saint Joseph Mercy Livingston Family Medicine Residency. His special areas of interest are  hospital medicine and medical ethics. He is a member of the American Academy of Family Physicians.

Anthony Vettraino, MD graduated from Michigan State University College of Human Medicine. He completed his family medicine residency at Providence Hospital. Dr. Vettraino has a special interest in the care of older patients and holds a Certificate of Added Qualification in Geriatrics. He is a member of the American Academy of Family Physicians, Michigan State Medical Society, and The American Geriatrics Society.
